It's your employer's responsibility to report your Temporary Disability Insurance benefits on your W-2 as Third-Party Sick Pay or Other Wages. If you have questions at tax time, talk to your employer. For Family Leave Insurance benefits, you'll need to download a 1099-G in January for the prior year.

When you complete your part online, you will get a unique Online Form ID number to give to your medical provider. They will use this number to submit their part to us online. You will be prompted to print out (or save as a PDF file) a page that has your unique Online Form ID number, along with instructions for your medical provider to follow. If you do not have access to a printer when initially filing your application, you have a fourteen day window to return and print the instructions when a printer is available. You can do so by clicking Access claim documents at the top of this page.

If planning ahead, you may start your online application up to 60 days in advance. To avoid deletion of your draft application: (1) within 14 days of starting the application, provide all information and confirm your claim; then (2) within 14 days of starting your leave, certify and file your claim.

Temporary Disability Insurance benefits are considered taxable income for purposes of both the federal income tax and FICA (Social Security). Federal income tax will not be withheld from your benefit payment each week unless you request it when you file, or submit IRS form W-4S to us (for state plan claims) or your employer (for private plan claims). Your share of FICA and medicare is automatically deducted from state plan benefit payments. Garnishments and any refunds owed from a previous disability or family leave claim will also be withheld from your benefit payments.

If your Temporary Disability benefits are not reported on your W-2, contact your employer directly to have it corrected. If needed, let your employer know they can download their year-end tax statement here. For additional information on incorrect W-2s, go to: Internal Revenue Service (IRS).

IF YOU RECEIVED PRIVATE PLAN TEMPORARY DISABILITY BENEFITS, OR NO DISABILITY BENEFITS: You will need to submit a new application for Family Leave Insurance benefits for bonding with your baby. Click here to start that process. If planning ahead, you may start your online application up to 60 days in advance. To avoid deletion of your draft application: (1) within 14 days of starting the application, provide all information and confirm your claim; then (2) within 14 days of starting your leave, certify and file your claim. If applying after your leave begins, you have 30 days from your first day of leave to file your application.

After the end of each calendar year, Form 1099-G will be available online for you to download and use when filing your federal income tax return. This form lists the total New Jersey State Family Leave Insurance (including Family Leave During Unemployment) benefits received that calendar year. This information is also sent to the Internal Revenue Service (IRS). If you receive Family Leave Insurance benefits from an approved private plan, your employer will provide you with the information required to file your federal income tax return. Family Leave Insurance benefits are not taxed by the State of New Jersey.

I Love is the fifth Korean extended play (seventh overall) by South Korean girl group (G)I-dle. It was released digitally on October 17, 2022, and physically on October 18, 2022, by Cube Entertainment. It consists of three physical versions: Born, Act, and X-file. There is also a jewel case for this album that consists of 5 different versions, which are the names of the members - Soyeon, Shuhua, Yuqi, Minnie and Miyeon. The EP contains six tracks, including the lead single "Nxde". The album talks about love and contains various kinds of love, and has many different emotions. The tracks in the EP are listed in reverse as it narrates the process of love starting with track 6 of someone experiencing love to track 1 of loving themselves. The EP took the form of vintage Hollywood-inspired concept.

The physical album was released in three versions with three concepts: Born, Act, and X-file. In Act version (G)I-dle exuded a provocative and seductive atmosphere with various heart-shaped objects. All the members are dressed in retro lingeries reminiscent of vintage silver screen divas and 1920s flappers.[22][48] This version portrays the double-sidedness of love, and the Born concept shows the visuals inspired by Hollywood figure Marilyn Monroe.[1] In the X-file version, the group shows their charms while covering their faces with a rabbit mask, creating an intimate and strange atmosphere that shows what lies underneath artists who are loved by everyone.[23][1] The EP comes with a CD, photo book, lyric sheets, two selfie cards, a bookmark, a sticker, a mini poster, a postage stamp, and a poster which is only available for pre-order.[49] The Target exclusive versions will contain an additional photo card.[50] The album also came in jewel cases as well, and there will be five versions covering all five members of the group.[51]

The group has created a podcast, launched first on Sept. 15, with a new episode to be released every two weeks thereafter until Thanksgiving. The podcast will include students reflections on what the group has experienced during the past number of years (wildfires, school shooting, Covid-19 pandemic and remote learning and the return to competitions) and how they got to be invited to perform in the parade. Each download of the podcast supports the group and raises funds so they can march in the parade. The podcast is available on Castro, Amazon Music, Apple Podcasts, Overcast, Podcast Addict and Spotify. Details on the podcast here: Donations may also be made here:
